If you continue having problems you may want to get a swallow study done on your boys. All 3 of mine ended up on thickened liquids due to them aspirating the thin liquids into their lungs. They don't always necessarily cough but it makes drinking VERY uncomfortable. This would make sense since you say sometimes they throw fits after only a few drinks and are becoming averse to the bottle. We had LOTS of feeding issues including reflux and aspiration so I understand your frustrations....
