Congratulations to your friend! Both Similac and Enfamil still offer the 3 cases/month for 3 months. The pediatrician needs to fill out the authorization form.

In our case, a nurse practitioner in the NICU pre-filled and signed the forms for us and I just added our address and mailed it.

With Enfamil, we then needed to call a hotline every month to confirm that we still wanted the next month's shipment. Similac just shipped monthly without any further need for us to contact Abbott.

each of my girls got a form from the NICU for a free case and I also called Similac (Neosure) and signed up for the multiple birth program and they sent 9 more cases so we ended up with 12 cases total. They're 9 months old now so should still work.

Worked for us, just wish we could always get the powder. We have gotten the ready-to-feed bottles twice and they only last a day.
