My little one (almost 10 months actual - 6.5 months adjusted)was originally rxd reglan and stopped in November, she is still on Zantac. This past Friday she started to have more severe reflux so I called the dr. to ensure her dosage was correct. Dr adjusted dosage but there is no improvement. She isn't in pain but does spit up a small amount hours after every feeding (except her last feed when she is down for 8+ hours). My pedi said this has to do with the stomach digesting and that going back on Reglan will help or I can just let her grow out of this phase. Pedi mentioned that adding rice cereal to my breastmilk and feeding more solids will help keep the food down. So, my question is do I go back on the Reglan? She isn't showing any signs of pain with the spit up and still does tummy time without problems. Has anyone been in this situation? What did you do? Thanks!!

Personally, I would just deal with the happy spitter for now. It is messy and stressful, but Reglan has so many restrictions on it, that I would use it only if you needed it. My DD was on it for about two months, and although it didn't work for us, I know it works for others. At 6.5 adjusted, you are right on the cusp of taking more solids, and things can improve quite a bit then.

On another note, if your DD is sitting up, crawling or scooting around more, this can actually cause a return of the spitup. Their trunk muscles still aren't very strong and they are often squishing their stomach and causing reflux...or if crawling, they are horizontal and the reflux that has been returning to their tummy now doesn't have gravity returning back where it should be. This too will get a bit worse until they start pulling up and cruising more.

I agree with Susan. There is a black label warning on reglan because it can cause neurological side effects. If she is not in pain and she is not losing weight, I would just deal with a little spit up- gladly.

From what I understand this is not a commonly used medication, one of my triplets was prescribed this and it did work for her, but bethanechol works similar to reglan where it speeds up gastric emptying and tightens the lower esophageal sphincter. It is not as risky with the side effects as reglan is. Also have they tried something like prevacid instead of zantac? Prevacid is a proton pump inhibitor and it works by decreasing the amount of acid in the stomach. Zantac is a H2 blocker and it works by making the acid less acidic. Zantac never did much for any of my trips, but prevacid helped tremendously for the other 2.
